St. Andrew's Place
Admissions
St. Andrew's Place (SAP) serves men and women age eighteen and older.
Appropriate candidates should meet the following criteria: (1) capable of
feeding him/herself (2) ambulatory (3) toilet trained and capable of dressing
and bathing (4) capable of safely living in a open, unfenced facility (5)
capable of learning domestic skills (6) limited history of aggression (7)
capable of and willing to attend a daytime program/employment. If the applicant
appears to meet the criteria they may apply for services through the MR/DD
HealthCare Authority and/or Department of Mental Health and Mental Retardation (DMH/MR)
and they will be placed on a waiting list. When an opening becomes available,
all other factors being equal, one's position on the waiting list will determine
their admission to the program. A recommendation as to admission will be made by
the Selection Committee to the Operations Director, who in turn will make the
final decision regarding admission. Other admission requirements apply as
determined by the DMH/MR and St. Andrew's Place policy. Privately funded
residents will be considered without distinction.
Placement is open to all persons categorized as mentally retarded by an ICAP
evaluation and psychological exam.
Services
St. Andrew's provides an age-appropriate environment as well as age appropriate
care and assistance.
Residents of St. Andrew's Place receive, at minimum, the following services:
housing, meals, staff supervision as needed for their level of independence,
training in domestic skills and job readiness, transportation, health
maintenance, counseling, and other services as needed to foster independence.
Individualized Service Plan
Each resident of St. Andrew's Place has an Individualized Service Plan (ISP)
which outlines one's strengths, weaknesses, and needs (goals/objectives) for the
coming year. The ISP is developed by an interdisciplinary team of professionals. St. Andrew's has a
team of Direct Service staff who are dedicated to ensuring maximum efficiency in
providing care and daily habilitation. Programming in
the residences is continuous throughout each day from waking and getting ready
for work to bathing, cooking, eating, relaxing, and doing household chores.
Resident Finances
In keeping with the goal of fostering independence each resident is assisted in
establishing a checking account.
St. Andrew's Place serves as payee for all third party payments (SSI, SSDI, VA,
etc.) and these payments are directly deposited in the individual resident's
account. Currently seventy-five percent of such payment is the resident's user
fee and is applied to the operating funds. The remaining twenty-five percent is
for the resident to use for personal spending.
